Balance Collection

On the Move will send an invoice prior to your visit or at your visit if IV Hydration services and cash is the preference of payment.

Patient:

• Understands that entire balance is to be collected prior or at appointment.

• Understands that any balance not collected will be sent to patient to pay immediately.

• Understands that no additional visits will be scheduled until past due balance is collected.

• Understands that a past due balance of 6 months or longer pay start the process into the collection process.
